Complete and sign the form. Your personal information is important to us. You have two options for filling out the application:<\/p>\n\n\n\n<p>Option 1) Download, print, and physically fill out the form. Then, scan, print, and mail the form to <a href=\"https:\/\/afm689.org\/contact.php\" target=\"_blank\" rel=\"noreferrer noopener\">our address<\/a>. We recommend this process for people who have never filled out a pdf digitally before.<br>Option 2) Digitally fill out the form using <a href=\"https:\/\/www.adobe.com\/acrobat\/pdf-reader.html\" target=\"_blank\" rel=\"noreferrer noopener\">Adobe Acrobat Reader<\/a> or your preferred application.
